Your Provider ID is located on the top left-hand corner of a VerifPoint / CreDENTALs Services credentialing letter or state application.
If you have never received a credentials verification request from VerifPoint or CreDENTALs Services before, please call a LastAPP Representative toll free today to receive your Provider ID at (888) 273-3368.
Utilizing LastAPP will allow you to centralize your credentialing information and help you respond to healthcare application or credentialing requests more efficiently.
